A new software tool continued to change the way we deliver primary care. The tool uses advanced data integration and predictive technology to identify patients at the highest risk of illness, letting us target preventive care to those who need it most. It’s so smart, in fact, that it can even predict how involved patients are likely to be in their own care, which helps us gauge when additional support may be needed. Our 45 Family and Community Medicine practices restructured more than a decade ago as medical homes focused on wellness and prevention.
